By the end of this course, participants will be able to:
• Understand the purpose and importance of effective incident investigation.
• Identify different types of workplace incidents (including near misses and environmental events).
• Apply structured investigation techniques to determine immediate, contributing, and root causes.
• Use tools such as the 5 Whys, Fishbone Diagram (Ishikawa), and TapRooT principles for root cause analysis.
• Conduct thorough interviews, gather physical evidence, and document findings objectively.
• Complete investigation reports in alignment with legal and ISO management system requirements.
• Recommend appropriate corrective and preventive actions aligned with the hierarchy of controls.
• Support continuous improvement and organizational learning from incidents.
